We have provided a few links below that you can follow which will provide you with more information in preparation of the vaccine in addition to the forms and documentation that we will require when you arrive at our clinic for your appointment.
Obtaining evidence of receiving the COVID-19 Vaccination Once you’ve had your vaccine, you’ll be able to get your immunisation history statement to prove your vaccination status. You can get proof of your vaccinations to include the COVID-19 Vaccine from your Medicare immunisation history by going through myGov website , or by calling Medicare on 1800 653 809.

If all possible, please complete and sign all documentation required, print it out and bring it to your appointment. This will help us tremendously to make your visit with us as smooth as possible.
